Instructions for Parents, Guardians, and Youth Group Leaders

All minors under age 18 are required to have a parent or legal guardian sign a waiver for all TOC events.

Option A:  If the parent or guardian e-signs the online waiver when registering the child on-line, no other action is needed, you do not need to fill out an offline form.

Option B:  If the parent or guardian does not register the child online, you may download, print, complete and sign the offline waiver, then bring the completed form to the event or scan it and e-mail it to the event director. If you forget, we have blank offline waivers at the event, it will just take a little longer to get you checked in.

Option C:  If someone other than the parent/guardian is accompanying the child to the event, such as a youth group leader or neighbor, make sure to bring the offline waiver completed and signed by a parent or guardian.
